OIT User Account Request Form

The Account Request Form should be completed by employee supervisors to obtain computer and system account access for their employees. Please do not use your browser's back button when navigating through the form.

If you have questions or concerns about the Account Request Forms or process, please contact the Help Desk at x64357 or by email at helpdesk@boisestate.edu .

Select the primary employee category for which the requested account request applies:

- Letter of Appointment must be approved prior to requesting accounts.

- HR EAF must be approved prior to requesting accounts.

- Accounts will expire at the end of the Spring semester and can be renewed by emailing the Help Desk prior to the end of the April. Departments should still submit a delete request when a student terminates employment.

- Emeriti requests are handled through the Provost's office.  Please contact for more details.

- Make sure you have submitted a 'Request for Approval of Affiliate / Visitor Status and Services' to HR prior to submitting your Account Request Form. Include this form with your account request. Departments must submit a delete request when the contractor no longer needs access.
